In this topic, you will learn how to sign a payee or servicer access request form as a senior officer.
Steps:
After the requestor signs their form, you will receive an email titled "Signature requested on "G5 Payee Access Form"."
Click the Review and sign link in that email.
Step 1Click the Verify Identity button to open ID.me.
Step 2ID.me will open in a new window.
To proceed, you must have a verified account in ID.me. For guidance on creating an ID.me account, please visit Creating Your ID.me Account.
Enter your email address and password for your ID.me account then click the Sign In button.
Step 4Enter the code provided by your multi-factor authentication option.
In this example, enter "326329" in the 6-digit code field.
Step 5Click the Continue button.
Step 6The G5 Payee Access Request Form will appear in Adobe Sign.
Click the Continue button to begin the signature process.
Step 7Click the Start button to begin editing fields.
Required fields are identified by a red asterisk(*).
Step 8Enter your name in the Authorized By field.
In this example, enter "John Pitt".
Step 9Enter your title in the Title field.
In this example, enter "Senior Director".
Step 10Enter your phone number in the Telephone # field.
In this example ,enter "2025555557".
Step 11Click in the Senior Officer Signature field to add your signature.
Step 12Your signature will in a popup window.
Click the Apply button to add your signature to the form.
Step 13Your signature now appears on the form.
Click the Click to Sign button to submit your signed form.
Step 14You will receive a confirmation that you have finished signing the form. You can also download a copy of the signed form.
